We work with health providers in a variety of industries to help ensure that the individuals we serve are given are given the best care possible when they face the loss of independence due to age or disability. We partner with physicians in a dual effort to provide both education and a comprehensive solution to any patient needs.

Become a Provider
Currently, LifeStream proudly works with over 60 provider partners. These partners come from a variety of industries, assisting in services such as homemaking, home health aide, nursing, and supplies.

Support or Sponsor Events
We invite health providers to partner with us to both participate in our events and refer clients that they believe may find value in one or many of these events. From conferences and seminars to golf outings and festivals, our events are designed to raise awareness and help to educate older adults and their loved ones about options for their health and overall wellness.
Some events that health providers have participated in or referred patients to in the past include, but are not limited to:

Speaker's Bureau
LifeStream’s Speakers Bureau can provide speakers for area organizations interested in learning more about the programs and services that we offer to the community. Our speakers are available for a variety of groups including, but not limited to, local clubs, businesses, hospitals/physicians offices, nursing home facilities, and universities. Some of the topics of focus include, but are not limited to:
- Wellness
- Transportation
- Volunteer Opportunities
- In-Home services
- Nutrition
- State Health Insurance Assistance Program (SHIP)
- Dementia Friendly Communities
