LifeStream Services and all sponsors strongly recommend each participant consult their physician with regard to preparation and competition in this program or any other physical activity. I, the undersigned participant, agree to hold harmless LifeStream and all sponsors, agents, employees, representatives, volunteers, and any other persons associated with the JOY Games for any action/claim which may result in injury incurred during my participation in the JOY Games. I warrant that I am in good physical health and able to compete in these events. I know of no physical restrictions which would prohibit my participation and have been advised by the sponsors that it would be in my best interest to consult my physician prior to participation. I understand preparation and competition may result in strenuous activity which could possibly activate any unrecognized, pre-existing condition I may have, thereby resulting in serious or life threatening physical harm to me. The sponsors have my permission to have a physician treat me, if needed, during my participation in the JOY Games. I agree and consent to the use of photographs and interviews to be used for publicity by LifeStream, and waive for claims for any compensation for such use.
