The process of seeking help as a caregiver be overwhelming. There are so many options out there, but only some that meet your loved one’s unique needs. Our Information and Assistance department can help to narrow down this search and guide you in the right direction. We provide a variety of options so that you may find the correct guidance for any obstacle that your loved one may face.

In-Home Care Management

Many find comfort in their own home, and because of this, keeping your loved one in their home, even as they age, becomes a priority. We look for ways to provide just that: the opportunity for seniors remain in their home. Our staff works diligently to assess your loved one’s need and assist wherever they can. This includes, but is not limited to:

  • Identifying funding sources for services
  • Providing our clients’ choices of in-home service providers in their area
  • Authorizing delivery Service
  • Client follow-up
  • Assistance with transitioning from nursing facilities back into a residential setting
  • Assistance with exploring options for care

Nutrition

Nutrition can play an essential role in one’s overall health. It can become incredibly tedious on caregivers to research the best diet for your loved one, then proceed to prepare regular meals. LifeStream’s Nutrition programs are designed to come alongside of you to help lessen your load.

Our team not only understands the nutritional needs of a senior, but they also provide options and services to ensure that seniors have access to these healthier meals and resources. Some of these nutrition centered programs and services include:

Wellness

A huge barrier that many caregivers face is the challenge of encouraging their loved one to focus more on overall wellness, whether that is managing their medication or pursuing a more active lifestyle. LifeStream recognizes the difficulty of this and works diligently to help provide safe options and resources focused on encouraging your loved one to be more active and health conscious. Dementia Friends

We understand the difficulty of caring for a loved one with dementia, and we also understand that much of this difficulty stems from culture’s perception of the illness. Dementia Friends Indiana is an outreach to combat just this. Its aim is to educate communities about dementia, break down the stigma of the disease, and implement practical changes to make life easier for both those trying to navigate the disease, and their loved ones.

Many caregivers have firsthand experience with this disease, and therefore, have the expertise to help drive this difference.

Caregiver Support

As a caregiver, you are not alone, and you shouldn’t have to navigate the difficulties of the task as though you are. Our caregiver support is made up of a variety of programs designed to assist caregivers whether they are caring for an elderly family member or they are grandparents raising their grandchildren. We offer multiple care options for support, from creating a space of open dialogue through support groups to making arrangements so that caregivers can have a little time for themselves. Some of our support options include:

Events

Our events provide a form of escape for many of our caregivers, and can also provide educational value for them in their roles. We put on a variety of events throughout the year, both virtually and in-person so that anyone who wants to participate can have the opportunity to do so, no matter what restrictions they may face in their responsibilities.
