Current Open Positions:

Responsible for assessing individuals’ needs providing information and referral services through the Aging & Disability Resource Center, writing and monitoring service plans, and utilizing resources within guidelines.  Work involves information and assistance, options counseling and community relation development. EOE.

Qualifications:  Bachelor’s degree from four-year college or university in social work, sociology, counseling, psychology, gerontology, valid IN RN license or a BA degree in any field with a minimum of two years full-time, direct service experience with the elderly or disabled which includes assessment, care plan development and monitoring; or a Master’s degree in a related field may substitute for the required experience. Licensed Social Work is preferred.  Options Counselors must meet experience requirements outlined by the Division of Aging for AD & TBI, Medicaid Waivers, SSBG/Title III, CHOICE, Pre Admission Screening.

LifeStream is seeking a part time time individual to work as a transportation driver in Blackford County. Driver will be responsible for providing safe and efficient transportation to riders.  This employee will work 16 hours per week on Tuesdays and Thursdays. Selected candidate is subject to DOT medical review and drug testing.  A satisfactory driving record is necessary. Please complete application online. EOE.

LifeStream is a non-profit organization working to improve the quality of life for people at risk of losing their independence.  As an In-Home Care Manager, you would assess client needs, provide information and referral services, write and monitor care plans, and utilize funds available to help clients stay in their own homes and/or communities in Madison County.

Qualifications:  Bachelor’s degree in social work, sociology, counseling, psychology, gerontology, or a BA degree in any field with a minimum of two years of experience working with the elderly or disabled which includes assessment, care plan development and monitoring; or a Master’s degree in a related field may substitute for the required experience.

Social Services / Care Management

  • Client intake/assessment to determine client needs & develop plan of care
  • Resource development & management to understand community resources
  • Case Management for in-home service clients to access necessary assistance to remain as independent as possible

Administration

  • Provide support in nonprofit administration duties
  • Answering phones, data entry, conference coordination, meeting organization, etc.
  • Attend local meetings with leadership or staff as available and drafting meeting notes.
